Social Workers (Permanent Roles) – Cromwell House Adult Community Mental Health Team. Are you a dedicated Social Worker looking for a rewarding career in mental health? We are hiring for two permanent Social Worker positions at Cromwell House Adult Community Mental Health Team.

About the Role

As a Social Worker in our team, you will play a key role in supporting adults with mental health needs. You will be part of a multidisciplinary team, working alongside healthcare professionals to deliver high-quality care and support.

Key Responsibilities:

  • Conduct assessments, support planning, reviews, and safeguarding duties under the Care Act 2014.
  • Work in compliance with the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
  • Act as a Care Coordinator under the Care Programme Approach (CPA).
  • Complete and update risk assessments.
  • Work collaboratively in a multidisciplinary team.
  • Promote anti-discriminatory practice and advocate for adults with care needs.
  • Prepare reports suitable for legal processes, including Mental Health Tribunals and Hospital Managers’ Hearings.
  • Take on or work towards the role of Approved Mental Health Professional (AMHP) or Best Interests Assessor (BIA) with full support for training and development.
